Since you globalized the scratch vm, maybe turbowarp extensions on scratch? I tried making it, it does work, but after saving the project, the project never loads. It should be the undefined hat block (?). Still figuring this out.

I've seen that issue with my projects using the extension too. I'm not sure why. I'll look into it and see if I can find anything. That's a great idea, though. By the way, is that Scratch Addons that's making that UI?

Fixed it! Turns out the ID of the extension was not recognized. There's an unused ID of "ScratchMath" that I used to replace it; now it works. However, your thing probably won't load correctly because all the TurboWarp extensions use unrecognized IDs too. There's not much we can do to fix this, sadly.
