contribute request: the create_table, insert values that create the chinook test data

Cool early contribution idea: add the create_table and insert commands that if run and if your database supported, would create the chinook database (see test folder) in your database

Note that Chinook comes with SQL for lots of different databases (theoretically there's a SQL standard, but the products all differ in some details of types and syntax). I just didn't check it in.
