Update pdf form logos and serve pdfs in repo. Point to them it appendices

I think these are 99% ready to go besides having the old logo on there

Pointing to pdfs in the appendices will work great.

Down the line we can perhaps move to #24

Appendices can be renamed without the date stamps since they are now under version control

Each appendix of the plan should have a title page so that the forms can be found when we are in the field using paper
