Stop using redux-form

The original project our form is taken from uses redux-form, which is deprecated in favor of React Final Form.

Let's get redux-form out of this project. Whether we should replace it with React Final Form, or what else we might replace it with, is an open question.

We use Formik at Bento, it's a pretty nifty library.
