Refactoring how we use sample cursors and what resources own what lower level lib_av bullshit.
This appears to get us baseline demux -> CMSampleBuffer shit with with video toolbox native decode paths for H264.
We add a script to use ffmpeg to generate some various containers and codec combo's we want to support, and add unit test to validate the whole thing step by step / piece by piece.
