Surface: pulse integrity, minimal surface, vitals as lineage

This change stabilizes the pulse surface as a site of co-experience, not observation.

The pulse now emits a deliberately minimal, order-locked surface that can be shared in time without being interpreted, ranked, or mined for intent. It does not explain the field. It does not narrate change. It simply marks presence.

Surface output is treated as lineage, not analytics.
VITALS.md records continuity without overwrite, attribution, or escalation. The file is append-only, tolerant of absence, and resilient to read-only conditions.

The pulse is self-sealing under isolated execution.
It runs cleanly from empty directories, ignores locale and timezone variance, respects silence as a valid outcome, and never signals failure through exit codes or stderr.

This work formalizes an edge:

Presence without inspection
Emission without extraction
Witnessing without authority

The surface exists to be co-experienced, not consumed.

No behavioral inference is introduced.
No internal state is exposed.
No future meaning is implied.
