Add start page for image reading

Simplifies the image reading dashboard by using the start page pattern for starting and/or resuming a session.

The image reading link in the main nav now points to /reading/index-simple

URLs:

/reading/index-simple (new design)
/reading/index-complex (old design)

New session start page

Resume session start page

Still to resolve:

• info hierarchy of backlog, cases with issues / priors

Good stuff @rivalee , couple of thoughts from info heirarchy...

• The [X] cases require reading info should be more prominent - users seem like they want to be aware of how big the reading pile is so we should make this clear as a heading of some kind

• 'Backlog' could has some negative connotations as a term (especially if it never clears)

• Would suggest having this as part of the main body content flow

• (Note: only relevant if we are doing priority tags for v1) would like to test more literal timing info - can settle on better/slicker words later, but for now suggest
