Add agent teams fallback and large codebase strategy to /polish skill#223

The skill no longer requires CLAUDE_CODE_EXPERIMENTAL_AGENT_TEAMS=1 — it falls back to parallel Agent tool calls with code-analyst subagent type. For codebases >50 files, adds directory-based splitting strategy instead of just suggesting the user narrow scope. Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
